The list looks pretty solid. There's only a few changes i can think of. First one that comes to mind would be -1 Tithe Drinker and +1 syndic of tithes. This is just personal preference but I always found syndic a bit more useful than the tithe drinker because of the easier casting cost and the additional toughness allows him to be a more efficient beater. Yes syndic doesn't have lifelink but at one toughness the drinker tends to find itself sitting back because you want to have the extra extort triggers. However with your large compliment of removal this may be a non-issue.
The other suggestion I have is maybe trying aven squire in the dutiful thrull slot. Its something I've seen a lot of other extort decks utilizing and it would also give your either another evasive beater or something that would help cover your tithe drinkers in combat by giving them that one extra toughness and keeping them from being traded with doomed travelers and pilgrims.
As for your sideboard the only thing I can think of is maybe +1 Keening Apparition, but that's probably just because I loathe the Junk enchantments deck.

I think I put in the Aven Squires (actually I think this is a brilliant idea for being a little more aggressive - the extort allows me to tank some more hits than usual so I might not need the Thrull defense and the squires increase the pressure) and get another Apparition for an undying evil (coming back in from a long magic break I have not much experience in the format atm so I am inclined to listen to your experience).
I agree with you on the Syndic: his Toughness is a big point in his favor. Although I can't bring myself to board out another Tithe drinker especially now that I have two Squires that might help them out. I am rather thinking one of the four Kingpin's pets to be easier on the curve also the Squires would fill its spot in the air so I think I can go without one of those.

I definitely agree with the poster above about the syndic. I would actually lower the number of kingpins pet/basilica guards and increase the syndics to 4 and tithe drinkers to 4. Replace a sign in blood or two with more removal (u want those tithe drinkers getting in there). Sign in blood is good, but 4 is unplayably slow. I think one maindeck undying evil is good measure. Also, 4 dead weight doesn't seem bad, the lower your spell curve, the more extorting you're going to do.

(IMO) If you're going to be an extort deck, be an extort deck. I wouldn't run either Thrulls or Squires in the deck, as a matter of fact the only non-extort creature I would run is Seraph of Dawn. You want your removal to be as cheap so you can get as many extort triggers as possible.
Sign in Blood should be kept at 4, it's your only way to dig out of trouble, and it's really cool to gain life and draw cards.

Yea I just disagree -- you don't want to play low value cards just because they happen to have extort -- you won't even be able to pay all of the extort triggers that are available to you most of the time. Kingpin/basilica guards are decent, i guess...
I think you're right about 4 sign in blood, I'm just worried that once people realize mono red aggro is a thing, the deck will be too slow.
